Experienced Social Worker - Adult Services Community Team

Our client based in North Somerset is seeking an experienced Social Worker to join their Adult Services Community Team. This is an excellent opportunity for a skilled practitioner to work with adults, carers, and families, supporting individuals to maintain their independence, wellbeing, and quality of life through strengths-based, person-centred practice.

The successful candidate will manage a varied caseload of adults with care and support needs, undertaking assessments, care planning, safeguarding interventions, and reviews. Working closely with health professionals, voluntary organisations, community services, and partner agencies, you will play a key role in coordinating support, managing risk, and ensuring individuals receive appropriate services to meet their identified needs and outcomes.

Responsibilities

  • Undertaking strengths-based assessments of adults with care and support needs in accordance with the Care Act 2014.
  • Managing a caseload of varying complexity, including vulnerable adults, older people, adults with disabilities, and those with complex social care needs.
  • Developing, implementing, and reviewing person-centred care and support plans that promote independence, choice, and wellbeing.
  • Completing risk assessments and ongoing reviews to ensure support remains appropriate and responsive to changing circumstances.
  • Undertaking safeguarding enquiries and interventions, ensuring adults are protected from abuse, neglect, and exploitation in line with statutory guidance.
  • Working collaboratively with individuals, carers, families, health professionals, and partner agencies to coordinate effective care and support arrangements.
  • Facilitating hospital discharges, identifying continuing healthcare needs, and supporting smooth transitions between services where required.
  • Promoting equal access to community resources, support services, and opportunities that maximise independence and reduce reliance on formal interventions.
  • Participating in duty functions, responding to referrals, enquiries, and urgent situations in a timely and professional manner.
  • Maintaining accurate and up-to-date case records, assessments, reports, and support plans in accordance with legislative and organisational requirements.
  • Contributing to multidisciplinary working and supporting service development through collaborative partnership approaches.

Qualifications and Experience

To be successful in this role you must have:

  • Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, CQSW, or equivalent recognised Social Work qualification.
  • Registration: Must be registered with Social Work England (SWE).
  • Experience: Significant post-qualified experience within Adult Social Care.
  • Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Adult Safeguarding legislation, and associated statutory guidance.
  • Assessment Skills: Proven experience completing Care Act assessments, reviews, risk assessments, and person-centred support planning.
  • Safeguarding Experience: Ability to undertake safeguarding enquiries and manage risk effectively within complex situations.
  • Multi-Agency Working: Experience working collaboratively with health colleagues, community services, voluntary organisations, and other partner agencies.
  • Communication Skills: Excellent verbal and written communication skills, including the ability to produce high-quality reports and assessments.
  • Professional Practice: Commitment to strengths-based, anti-discriminatory, and person-centred practice that promotes positive outcomes for adults and carers.
  • Driving Licence: Full UK driving licence and access to a vehicle is desirable due to the community-based nature of the role.
