Social Worker - Cognitive and Physical Disability (C&PD)

Salary: Between £32,070 – £34,359 for Newly Qualified Social Workers, £35,451 – £39,165 for Level 2 Social Workers and £39,939 - £42,699 for Level 3 Social Workers (plus £3,300 market premia payment for Level 3 Social Workers) – Pay Award Pending
Working Hours: Monday to Friday, 37 Hours Per Week
Contract Type: Permanent & Secondment

About The Role
The team is knowledgeable, friendly and supportive, with a wealth of contacts and experience amongst the extensive Physical and Cognitive community and provider network within Charnwood and Melton. They have weekly Group Supervision Meetings to promote the best possible outcomes for the people we work with. All staff benefit from regular manager supervision, manageable workloads and support from specialist teams within the department (such as finance or legal services).
We provide a comprehensive induction package with ongoing access to bespoke learning and development opportunities. This includes tailored continuous professional development sessions; opportunities for supported further qualifications and access to academic research and practice expertise to develop skills, experience and inform our employees’ practice.
Our Service
Support for adults with physical disabilities and older people with eligible needs related to frailty and cognitive disabilities, arising from long term conditions such as Dementia, Stroke, Parkinson’s Disease and Brain Injury. The service will support adults with early onset dementia including alcohol related dementia where the primary need is linked to needs arising from dementia rather than alcohol dependency. Longer- term and more complex work will be done in the C&PD teams, with short term interventions being the responsibility of the Home First service.
The focus of C&PD work is to promote independence and opportunities with a focus on outcomes that promote wellbeing.
Locality based C&PD teams include Community Support Worker, Social Worker and AHMP posts and will be responsible for Care Act Assessment, Safeguarding, Mental Capacity Assessments/Best Interests decisions and Court work.

About You
To apply for this post, you must:
- Have a degree in Social Work approved by the Health Care Professions Council (HCPC) or equivalent social work qualification ie Certificate of Qualification in Social Work or Diploma in Social Work, approved by HCPC. Be a qualified Social Worker and registered with Social Work England.
- Have knowledge of social work methodologies, social care policies such as person-centred approaches and asset based social work, and relevant legislation including The Care Act, Mental Health Act, and Mental Capacity Act.
- Have experience of effective assessment and risk management delivery, safeguarding experience, court work experience and multi-agency/multidisciplinary working and/or hospital discharge processes is essential.
- Ideally have experience of working directly with people with Learning Disability and Autism.
- Have experience of partnership working with adults to identify and meet need with creative and pragmatic solutions.
- Be a confident communicator who is skilled in diplomacy, persuasion, assertive and conflict management and able to build effective working relationships and be able to work proactively individually but also as a team member.
- Have good prioritisation and time management skills and have a valid UK driving licence or access to a vehicle
A DBS enhanced check for a regulated activity is required for this post.
