We are looking for an enthusiastic and compassionate Social Worker to join our Mansfield Community Social Work Team within Adult Social Care.
This is an excellent opportunity for a social worker who is passionate about helping adults live independently, achieve their goals, and stay connected to their communities. You will receive support, supervision, and development opportunities, and flexible hybrid working arrangements.
As a Social Worker, You Will
- Assess the needs of adults and develop person-centred support plans.
- Promote independence, wellbeing, choice, and control.
- Use a strengths-based approach to help people achieve their goals and reduce reliance on long-term services.
- Identify reablement and enablement opportunities to support recovery and independence.
- Work closely with individuals, carers, families, communities, and partner organisations.
- Carry out reviews and monitor support arrangements to ensure positive outcomes.
- Undertake safeguarding work and develop skills in applying the Mental Capacity Act.
- Contribute to multi-disciplinary working, including discharge planning when required.
- Build strong relationships with health colleagues, voluntary organisations, and community groups.
- Maintain accurate records and reports.

You Will Have
- A Social Work qualification recognised by Social Work England.
- Registration with Social Work England.
- Knowledge of adult social care legislation and current policy and practice.
- Excellent communication and relationship-building skills.
- Strong organisational skills and the ability to manage your own varied workload.
- Experience of working with adults and supporting positive outcomes.
- Good IT skills and the ability to maintain accurate records.
- A commitment to strengths-based practice and person-centred support.
- A full driving licence and access to transport (reasonable adjustments will be considered for disabled applicants).
