Experienced Social Worker - Locality Team 2
Come to Merton and make a difference.
We are looking for an experienced Social Worker to join Locality Team 2 within Adult Social Care at Merton Council.
This is an opportunity to work in a supportive and experienced team that values good practice, professional development and staff wellbeing. You will work with adults with care and support needs, helping them maintain their independence, remain safe and achieve positive outcomes.
The Role Includes
- Completing strengths-based assessments, reviews and care and support plans.
- Managing a varied caseload.
- Undertaking safeguarding work and risk management.
- Working closely with families, carers and partner agencies.
- Supporting hospital discharge and community-based interventions.
- Promoting independence and preventing or delaying the need for ongoing support.

We Are Looking For Someone Who
- Is a qualified and registered Social Worker.
- Has experience within Adult Social Care.
- Has sound knowledge of the Care Act, Mental Capacity Act and safeguarding practice.
- Can manage complex cases and make informed decisions.
- Has strong communication and assessment skills.
- Works collaboratively and builds positive relationships with residents, colleagues and partners.
