Job Opportunity: Social Worker - Home First Neighbourhood Vale Team
Liquid Personnel is recruiting an experienced Social Worker for its client's Home First Neighbourhood Vale Team based in Oxfordshire. This is a full-time agency role supporting adults transitioning from acute and community hospital settings back into their local communities through safe, timely, and person-centred discharge planning.

Your Responsibilities
As a Social Worker within the Home First Neighbourhood Vale Team, you will:
- Support safe and effective hospital discharges from acute and community settings.
- Complete Care Act Assessments, Mental Capacity Assessments, and Best Interest Decisions.
- Develop personalised care and support plans that promote independence and wellbeing.
- Work collaboratively with health professionals, carers, families, and partner agencies.
- Attend Multi-Disciplinary Team (MDT) meetings and contribute to decision-making.
- Advocate for individuals and ensure their rights, wishes, and outcomes remain central to practice.
- Manage complex cases, assess risks, and respond appropriately to safeguarding concerns.
- Support adults with mental health needs through strengths-based interventions.
- Apply relevant legislation including the Care Act 2014, Mental Capacity Act 2005, and Mental Health Act.
- Maintain accurate and timely case records, assessments, and reports.

Essential Requirements
- Social Work qualification (BA, MA, BSc, DipSW, CQSW or equivalent).
- Registered with Social Work England.
- Significant post-qualified experience within Adult Social Care.
- Experience in assessment, support planning, safeguarding, and hospital discharge work.
- Strong knowledge of the Care Act 2014, Mental Capacity Act 2005, Mental Health Act, and safeguarding legislation.
- Ability to undertake strengths-based assessments and deliver outcome-focused support plans.
- Experience working effectively within multi-agency settings.
- Excellent communication, organisation, and report-writing skills.
- Full UK driving licence and access to a vehicle is desirable.
