About the Role
As part of the Hospital Integrated & Community Discharge Team, you will support adults through the discharge process, helping to ensure safe and timely transitions from hospital settings into the community.
Key responsibilities include:
- Working across acute hospitals, non-acute hospitals and community settings
- Supporting the delivery of the Discharge to Assess model to ensure effective and timely hospital discharges
- Completing strengths-based assessments and care planning to promote independence and wellbeing
- Collaborating closely with health professionals, council colleagues and partner agencies to achieve person-centred outcomes
- Ensuring available resources are utilised effectively to meet assessed needs
- Working in accordance with relevant legislation including the Care Act, Mental Capacity Act and Mental Health Act
- Maintaining high professional standards while delivering effective social work intervention
- Promoting safeguarding and ensuring the welfare and protection of vulnerable adults at all times
About You
To be considered for this position, you will have:

- A recognised Social Work qualification (Degree, DipSW, CQSW or equivalent)
- Registration with Social Work England
- A minimum of 2 years' post-qualifying Social Work experience
- Experience within Adult Social Care, hospital discharge or community-based services
- Sound knowledge of the Care Act, Mental Capacity Act and Mental Health Act
- Strong assessment, communication and partnership-working skills
- A commitment to person-centred and strengths-based practice
- A full UK driving licence and access to your own vehicle
