Job Opportunity: Social Worker in Walsall

Our client based in Walsall is seeking an experienced Social Worker to join their Intermediate Care Services Team. This is an excellent opportunity for a skilled Adult Social Care practitioner to support adults with a range of care and support needs through person-centred, strengths-based interventions that promote independence, wellbeing, and positive outcomes.

Working within a fast-paced multidisciplinary environment, you will be responsible for undertaking Care Act assessments, reviews, safeguarding enquiries, and Mental Capacity Assessments. The successful candidate will manage a varied caseload, working collaboratively with service users, carers, health professionals, and partner agencies to ensure individuals receive timely and effective support that maximises their independence and reduces the need for long-term services wherever possible.

Key Responsibilities

  • Undertaking strengths-based, person-centred Care Act 2014 assessments to identify eligible care and support needs.
  • Completing reviews of existing care and support arrangements to ensure services remain appropriate, effective, and outcome-focused.
  • Developing and implementing personalised care and support plans that promote independence, choice, and wellbeing.
  • Carrying out Mental Capacity Assessments and contributing to Best Interest decision-making in accordance with the Mental Capacity Act 2005.
  • Undertaking safeguarding enquiries and responding appropriately to allegations of abuse, neglect, self-neglect, and other safeguarding concerns.
  • Managing a varied adult social care caseload and prioritising work effectively in a busy Intermediate Care setting.
  • Working collaboratively with health colleagues, service users, carers, families, and partner organisations to deliver coordinated care and support.
  • Applying strengths-based practice to support individuals to achieve their personal outcomes and maximise independence within their communities.
  • Assessing and managing risk, ensuring robust decision-making and proportionate interventions.
  • Maintaining accurate, timely, and high-quality case records, assessments, reports, and support plans in line with statutory and organisational requirements.
  • Participating in multidisciplinary meetings and contributing to integrated approaches to assessment, discharge planning, and service delivery.

Qualifications & Experience

  • Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW.
  • Registration: Must be registered with Social Work England (SWE).
  • Experience: Significant post-qualified experience within Adult Social Care, including assessments, reviews, safeguarding, and support planning.
  • Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, and Adult Safeguarding legislation.
  • Assessment Skills: Experience completing strengths-based assessments, Mental Capacity Assessments, reviews, and outcome-focused support plans.
  • Safeguarding Experience: Proven ability to undertake safeguarding enquiries and manage risk effectively.
  • Multi-Agency Working: Experience working collaboratively with NHS professionals, carers, families, and partner agencies.
  • Skills: Excellent communication, assessment, analytical, report-writing, and case management skills with the ability to work independently and manage competing priorities.
  • Driving Licence: Full UK driving licence and access to a vehicle is desirable.
