Liquid Personnel is recruiting an Adult Social Workers to join a dedicated Learning Disabilities Review Project in Taunton, for our client.
This is an excellent opportunity to work within a supportive multi-disciplinary environment, undertaking Care Act reviews and reassessments to ensure adults receive the right support and achieve positive outcomes.
What will your responsibilities be?
- Undertake Care Act reviews and reassessments.
- Complete face-to-face visits in community, supported living and residential settings.
- Produce high-quality, evidence-based assessments, reviews and support plans.
- Identify and evidence eligible care and support needs.
- Contribute to Continuing Healthcare (CHC) processes and funding discussions.
- Work collaboratively with multidisciplinary teams and partner agencies.
- Maintain accurate and timely case records.
- Manage your own workload and deliver outcomes within agreed timescales.
Working Arrangements
This is a hybrid role requiring attendance in a Somerset office a minimum of two days per week, with travel across the county to attend visits and assessments.

Qualifications and Experience
To be successful in this role you must have:
- Qualified Social Worker with a recognised Social Work degree or equivalent.
- Current Social Work England registration.
- Significant post-qualified Adult Social Care experience.
- Strong background in Learning Disabilities services.
- Experience completing Care Act reviews and reassessments.
- Excellent assessment, analytical and report-writing skills.
- Knowledge of CHC processes, joint funding arrangements and health/social care boundaries.
- Ability to work independently and make defensible professional decisions.
- Full UK driving licence and access to a vehicle.
