Social Worker – Mental Health

Job Title

Our client based in Neath Port Talbot is seeking an experienced Social Worker to join their Mental Health Team. This is an excellent opportunity for a skilled practitioner to work with adults experiencing mental health needs, supporting them to achieve positive outcomes, maximise independence, and improve their wellbeing through strengths-based and person-centred practice.

The successful candidate will work closely with individuals, carers, families, health professionals, and partner agencies to assess needs, coordinate services, manage risk, and develop effective support plans. Working within a multidisciplinary environment, you will play a key role in safeguarding vulnerable adults, promoting recovery, facilitating access to community resources, and ensuring individuals remain at the centre of all decision-making.

This role requires a confident Social Worker with strong assessment skills, experience of complex case management, and a comprehensive understanding of mental health practice, safeguarding, capacity assessments, and partnership working.

Responsibilities

In this role, you will:

• Managing a complex caseload of adults with mental health needs, ensuring timely and effective social work interventions.
• Undertaking strengths-based assessments in partnership with individuals, families, carers, and professionals to identify needs, risks, and desired outcomes.
• Developing, implementing, coordinating, and reviewing person-centred care and support plans that promote recovery, independence, and wellbeing.
• Identifying and utilising community resources, natural support networks, and local services to support individuals in achieving their outcomes.
• Working collaboratively with health professionals, partner agencies, carers, and community organisations to deliver coordinated care and support.
• Undertaking Mental Capacity Assessments and facilitating Best Interest decision-making in accordance with the Mental Capacity Act.
• Completing Continuing Healthcare (CHC) assessments and contributing to multidisciplinary eligibility processes where required.
• Undertaking safeguarding enquiries and initiating appropriate action to protect adults at risk from abuse, neglect, exploitation, and self-neglect.
• Carrying out risk assessments and applying Positive Risk Taking principles to support individuals safely and effectively.
• Conducting non-criminal investigations as part of Adult at Risk processes and safeguarding procedures.
• Attending and contributing to case conferences, multidisciplinary meetings, reviews, and safeguarding meetings, providing a professional social work perspective.
• Chairing reviews, meetings, and case conferences where required.
• Undertaking financial assessments in accordance with relevant charging frameworks and policies.
• Supporting hospital discharge planning and community-based interventions to prevent unnecessary admissions where appropriate.
• Maintaining accurate, timely, and legally compliant case recording, assessments, reports, and documentation.
• Contributing to service development, team learning, continuous improvement initiatives, and professional development activities.
• Supporting colleagues with complex casework and providing guidance on high-risk situations and interventions where required.
• Undertaking Approved Mental Health Professional duties if qualified and required by the service.
