Adult’s Social Worker
Liquid Personnel is recruiting an Adult’s Social Worker to join its client’s Mental Health Discharge team, based in Shropshire. This is an excellent opportunity for a dedicated practitioner to support adults with complex mental health needs through safe, timely, and person-centred discharge planning.
The successful candidate will work closely with service users, carers, health professionals, housing services, and partner agencies to enable successful transitions from hospital into the community. You will play a key role in coordinating care, managing risk, promoting recovery, and ensuring individuals receive the right support to maximise independence and wellbeing following discharge.
Responsibilities
- Managing a caseload of adults with complex mental health needs, ensuring effective and timely discharge planning.
- Undertaking Care Act 2014 assessments, reviews, and support planning to identify eligible care and support needs.
- Applying the Mental Health Act 1983/2007 and Mental Capacity Act 2005 within day-to-day practice and decision-making.
- Supporting safe and timely discharge from inpatient mental health settings by identifying and addressing barriers to discharge.
- Coordinating Section 117 aftercare planning and reviews, ensuring eligible individuals receive appropriate aftercare services and support.
- Arranging and coordinating care packages, supported accommodation, placements, and community support services to meet assessed needs.
- Undertaking Mental Capacity Assessments and facilitating Best Interest decision-making where required.
- Assessing and managing risk effectively, balancing safety, recovery, independence, and positive risk-taking.
- Undertaking safeguarding enquiries and responding to concerns relating to abuse, neglect, self-neglect, and exploitation.
- Working collaboratively with Community Mental Health Teams, health professionals, carers, housing providers, statutory agencies, and voluntary organisations.
- Attending multidisciplinary meetings, discharge planning meetings, safeguarding discussions, and case reviews.
- Providing advice, guidance, and support to service users, carers, colleagues, and partner agencies.
- Maintaining accurate, timely, and legally compliant assessments, reports, support plans, and case records.

Qualifications and Experience
- Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, CQSW, CSS, or equivalent recognised Social Work qualification.
- Registration: Must be registered with Social Work England.
- Experience: Significant post-qualified experience within Adult Social Care or Adult Mental Health Services.
- Mental Health Experience: Experience working with adults experiencing complex mental health needs, including discharge planning and community-based support.
- Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Health Act 1983/2007, Mental Capacity Act 2005, Section 117 aftercare responsibilities, Adult Safeguarding legislation, and associated statutory guidance.
- Assessment Skills: Proven ability to undertake complex assessments, support planning, risk assessments, and discharge planning.
- Safeguarding Experience: Experience undertaking safeguarding enquiries and managing safeguarding concerns involving vulnerable adults.
- Multi-Agency Working: Strong experience working collaboratively with health services, housing providers, community organisations, carers, families, and partner agencies.
- Communication Skills: Excellent communication, report-writing, analytical, negotiation, and decision-making skills.
- Professional Practice: Commitment to strengths-based, recovery-focused, and person-centred social work practice.
