Senior Social Worker (Mental Health Team) – Community-Based Role
Location: Cardiff Contract Type: Monthly/Temporary Sector: Adult Mental Health & Social Care
About the Role
Our client, based in Cardiff, is seeking an experienced Social Worker to join their Mental Health Team. Based in a community setting, this role will involve supporting adults with complex mental health needs, focusing on promoting independence, recovery, and wellbeing.
You’ll be responsible for assessing, planning, and reviewing care and support, managing risks, and safeguarding adults in line with statutory obligations. Working within a multi-disciplinary team, you’ll collaborate with health services, external agencies, and support networks to deliver joined-up, person-centred care for individuals and their families.
Key Responsibilities
Core duties include:
- Managing a caseload of adults with mental health needs, including those with complex or high-risk cases.
- Conducting comprehensive assessments to identify needs, risks, and suitable interventions.
- Developing, implementing, and reviewing care and support plans to encourage recovery and independence.
- Working within relevant legislation and safeguarding frameworks to protect vulnerable adults.
- Supporting individuals to understand their circumstances and make informed decisions about their care.
- Ensuring families, carers, and support networks are involved and their needs are met.
- Collaborating with multi-disciplinary teams, including health professionals and partners, for coordinated support.
- Undertaking and managing risk assessments, ensuring proportionate, defensible decisions.
- Balancing workloads, prioritising tasks, and maintaining professional judgement.
- Attending case conferences, reviews, and decision-making forums.
- Maintaining accurate, detailed, and compliant case records.
- Contributing to service improvement, team development, and best practice.
- Providing informal support or guidance to less experienced colleagues.
- Aligning care planning within budget frameworks to ensure cost-effective solutions.

Qualifications & Experience
Essential Qualifications
- Degree in Social Work (or equivalent qualification).
- Registration with Social Care Wales.
Experience (Minimum 2 years post-qualification required)
- Experience working with adults experiencing mental health difficulties.
- Proven experience managing complex caseloads and high-risk situations.
- Experience in:
- Mental health assessments and care planning.
- Risk assessments and safeguarding adults at risk.
- Ongoing support planning and reviews.
- Community or multi-disciplinary mental health environments.
- Working with carers and wider support networks.
Legislation & Policy
- In-depth knowledge of:
- Mental Health legislation: Act 2007 & relevant updates.
- Care Act 2014 (assessments and safeguarding).
- Mental Capacity Act 2005.
- Safeguarding Adults procedures.
- Relevant social care regulations.
Key Skills
- Strong assessment and risk evaluation in a mental health context.
- Ability to manage a varied, high-pressure workload efficiently.
- Professional, non-judgemental, excellent communication skills.
- Experience working in multi-disciplinary environments.
- Accurate case note-keeping to professional standards.
- Skilled in policy advocacy for service users.
- Orchestran skills for collaboration, decisions, and resilience.
- Proactive, solution-focused ethos.
- Strong budget awareness for support planning.
