About the Role

Adult Social Care currently have a Social Worker vacancy to join our Mental Health Hospital Discharge Countywide team. You will work with working age adults (18-70) and older adults (70+) and will be supported by a team that cares for you, just as much as it cares for our community.

We are looking for a Social Worker to support effective and timely hospital discharges. You will work across East Sussex covering all acute hospitals for people who have been admitted to hospital to seek treatment for their mental health. This is a new enterprise, you will work as part of a remote partnership team with Sussex Partnership Foundation Trust (SPFT) and the voluntary sector together with the Adult to identify safe, creative discharges, placing the Adult at the centre.

You will be collaborating closely with other stakeholders, working together to achieve the utmost discharge plans.

Requirements

We are looking for candidates who:

  • Are Social Worker registered with Social Work England, please include your registration number on your application. Who has completed their assessed & supported year in employment (ASYE).
  • Can build positive relationships with clients, family members and professionals.
  • Are self-motivated.
  • Are able to meet travel requirements of the post.
  • Can show a commitment to training and personal development.

Hybrid working arrangements are applicable to the role, this includes elements of office-based working, client visits and working from home.
