Adult’s Social Worker

Liquid Personnel is recruiting an Adult’s Social Worker to join its client’s North Wellbein team, based in Stoke-on-Trent. This is an excellent opportunity for a dedicated practitioner to support adults aged 18 and over, primarily those with physical disabilities and long-term care and support needs, through strengths-based and person-centred social work practice.

The successful candidate will manage a varied community-based caseload, undertaking Care Act assessments, reviews, safeguarding interventions, and support planning to help individuals maximise their independence and wellbeing. Working closely with service users, carers, health professionals, and partner agencies, you will play a key role in ensuring adults receive appropriate support whilst promoting choice, control, and positive outcomes.

This role requires a confident Social Worker who can work independently, undertake community visits, participate in the duty rota, and manage competing priorities within a busy Adult Social Care service.

Responsibilities

  • Managing a caseload of adults aged 18+ with physical disabilities, long-term conditions, and complex care and support needs.
  • Undertaking Care Act 2014 assessments, reviews, reassessments, and support planning to identify eligible needs and achieve positive outcomes.
  • Developing, implementing, and reviewing person-centred Care and Support Plans that promote independence, wellbeing, and choice.
  • Undertaking Mental Capacity Assessments and contributing to Best Interest decision-making in accordance with the Mental Capacity Act 2005.
  • Responding to safeguarding concerns and undertaking safeguarding interventions in line with Adult Safeguarding legislation and local procedures.
  • Assessing and managing risk appropriately, balancing individual choice with safety and positive risk-taking.
  • Participating in the team duty rota, responding to referrals, urgent situations, and undertaking duty visits where required.
  • Conducting home visits and community-based assessments to ensure individuals receive timely and effective support.
  • Working collaboratively with health professionals, community services, care providers, carers, families, and partner agencies to coordinate services and support.
  • Promoting strengths-based practice and supporting individuals to access community resources, informal networks, and preventative services.
  • Maintaining accurate, timely, and legally compliant records using the Liquidlogic case management system.
  • Participating in multidisciplinary meetings, safeguarding discussions, reviews, and case conferences as required.

Qualifications and Experience

  • Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, CQSW, CSS, or equivalent recognised Social Work qualification.
  • Registration: Must be registered with Social Work England.
  • Experience: Significant post-qualified experience within Adult Social Care services.
  • Community Experience: Experience supporting adults with physical disabilities, long-term conditions, and complex care and support needs within a community setting.
  • Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Adult Safeguarding legislation, and associated statutory guidance.
  • Assessment Skills: Proven ability to undertake Care Act assessments, reviews, support planning, and risk management interventions.
  • Safeguarding Experience: Experience undertaking safeguarding enquiries and managing safeguarding concerns.
  • Systems Knowledge: Experience using Liquidlogic or similar Adult Social Care recording systems is desirable.
  • Multi-Agency Working: Strong experience working collaboratively with health professionals, community services, care providers, families, carers, and partner agencies.
  • Communication Skills: Excellent communication, report-writing, analytical, negotiation, and decision-making skills.
  • Professional Practice: Commitment to strengths-based, person-centred practice and continuous professional development.
  • Flexibility: Ability to attend the office a minimum of two days per week, participate in the duty rota, and undertake community visits as required.
