Liquid Personnel is recruiting three experienced Social Workers

Liquid Personnel is recruiting three experienced Social Workers for its client's Preparation for Adulthood (PFA) Team in Barnsley. (Competitive Hourly Rate)

This is an exciting opportunity to support young people aged 14-25 as they transition into adulthood, helping them maximise independence, achieve positive outcomes, and access the support they need for the future.

Working across both Children's and Adult Social Care, you'll play a key role in delivering strengths-based assessments, transition planning, and person-centred support for young people with a range of additional needs.

What will your responsibilities be?

  • Support young people aged 14-25 through transition into adulthood.
  • Complete assessments, reviews, and support plans using a strengths-based approach.
  • Work across both Children's and Adult Social Care legislation.
  • Promote independence, choice, and positive long-term outcomes.
  • Contribute to Education, Health and Care Plans (EHCPs) and transition planning.
  • Work closely with young people, families, carers, education providers, health professionals, and partner agencies.
  • Support young people with SEND and complex needs.
  • Develop creative solutions and support pathways to maximise life opportunities.
  • Contribute to the ongoing development of the Preparation for Adulthood service.

Qualifications & Experience:

To be successful in this role you must have,

  • Qualified Social Worker (BA, MA, DipSW, CQSW or equivalent).
  • Registered with Social Work England.
  • Experience working with young people, adults with disabilities, or transition services.
  • Strong knowledge of the Care Act 2014, Children Act 1989, Mental Capacity Act 2005, and Human Rights legislation.
  • Understanding of SEND, EHCPs, safeguarding, and Continuing Healthcare.
  • Excellent assessment, care planning, and multi-agency working skills.
  • Passion for supporting young people to achieve independence and positive outcomes.
