Social Worker - Preparation for Adulthood Team

Liquid Personnel is recruiting Social Worker for its client's Preparation for Adulthood Team in Dorset.

This is an exciting opportunity to work within a specialist Birth to Settled Adulthood Service, supporting young people with disabilities and their families as they prepare for adult life.

Working within a strengths-based, multidisciplinary team, you will collaborate with colleagues across social care, health, SEND, and education to support young people aged 14 to 25. The role has a unique focus on preparation for adulthood, helping young people build independence, develop life skills, and achieve their aspirations as they transition towards adulthood.

The successful candidate will primarily complete and review Preparation for Adulthood Plans, working directly with young people and their families to identify goals and support pathways that maximise independence and promote positive outcomes. Whilst this role does not involve long-term case holding, there may also be opportunities to undertake Care Act assessments, support planning, Mental Capacity Assessments, and Best Interest decision-making where required.

This role requires a motivated practitioner who is passionate about supporting young people through transition and ensuring that adulthood is approached in a planned, informed, and person-centred way.

What will your responsibilities be?

• Completing and reviewing Preparation for Adulthood Plans for young people aged 14 to 25 with disabilities and additional needs.
• Working directly with young people and their families to identify aspirations, strengths, and goals that support a successful transition into adulthood.
• Promoting independence skills, self-advocacy, community participation, employment opportunities, education, and independent living.
• Using strengths-based and person-centred approaches to support young people in achieving positive long-term outcomes.
• Collaborating closely with professionals across social care, health, SEND, education, and partner agencies to ensure coordinated support.
• Undertaking Care Act assessments and Care and Support Planning where required as part of transition planning.
• Completing Mental Capacity Assessments and facilitating Best Interest decision-making processes in accordance with the Mental Capacity Act 2005.
• Supporting young people and families to understand available resources, services, support options, and transition pathways.
• Providing professional advice and guidance to colleagues regarding preparation for adulthood principles and approaches.
• Contributing to the development of good practice in transition planning and promoting independence across the wider service.
• Participating in multidisciplinary meetings, reviews, planning discussions, and transition-related forums.
• Maintaining accurate, timely, and legally compliant records, assessments, plans, and reports.
• Supporting a smooth transition between children's and adult services, ensuring there are no unnecessary gaps in support.
