Job Opportunity: Social Worker in Leicester
Our client based in Leicester is seeking an experienced Social Worker to join their Preparing for Adulthood Team. This is an excellent opportunity for a dedicated practitioner to support young people as they transition from children's services into adulthood, ensuring they achieve the best possible outcomes whilst maximising independence, wellbeing, and choice.
The successful candidate will work closely with young adults, their families, carers, education providers, health professionals, and partner agencies to develop person-centred plans that support a smooth transition into adult services. You will be responsible for undertaking Care Act assessments, safeguarding work, Court of Protection processes, and supporting young people with complex needs, including those receiving Continuing Healthcare (CHC) and shared care arrangements.
This role requires a confident practitioner who can work in a strengths-based way, manage risk effectively, and support young adults to develop the skills and support networks needed to live fulfilling and independent lives.
Key Responsibilities
Core duties include:
- Undertaking Care Act 2014 assessments for young adults transitioning into Adult Social Care services.
- Supporting young people and their families through the transition from children's placements and services into adulthood.
- Developing and reviewing person-centred Care and Support Plans that promote independence, wellbeing, and positive outcomes.
- Managing a caseload of young adults with complex care and support needs, ensuring timely assessments and interventions.
- Undertaking safeguarding enquiries and responding appropriately to concerns relating to abuse, neglect, exploitation, or self-neglect.
- Supporting Court of Protection processes, including the preparation of reports, assessments, and supporting documentation where required.
- Completing Mental Capacity Assessments and contributing to Best Interest decision-making in accordance with the Mental Capacity Act 2005.
- Working closely with families, carers, education providers, health professionals, and partner agencies to coordinate effective support arrangements.
- Developing a strong understanding of Continuing Healthcare (CHC), shared care arrangements, and transition planning to ensure seamless service provision.
- Undertaking strengths-based risk assessments that balance safety with independence, choice, and control.
- Promoting independence, resilience, and community inclusion, ensuring young adults are supported to achieve their personal goals and aspirations.
- Maintaining accurate, timely, and high-quality case records, assessments, reports, and support plans.

Qualifications & Experience
- Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW.
- Registration: Must be registered with Social Work England.
- Experience: Significant post-qualified experience within Children's, Adult, or Transition Services.
- Transition Experience: Experience supporting young people through Preparing for Adulthood and transition processes.
- Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Court of Protection processes, Adult Safeguarding legislation, and Continuing Healthcare frameworks.
- Assessment Skills: Proven ability to complete Care Act assessments, Mental Capacity Assessments, risk assessments, and support planning.
- Court of Protection Experience: Experience supporting Court of Protection applications and related legal processes.
- Safeguarding Experience: Experience undertaking safeguarding work and managing complex risk situations.
- Multi-Agency Working: Strong experience working collaboratively with families, health professionals, education providers, and partner agencies.
- Communication Skills: Excellent assessment, communication, report-writing, and relationship-building skills.
- Professional Practice: Commitment to strengths-based, person-centred practice that promotes independence and positive outcomes for young adults.
