Join Our Community Learning Disabilities Team
Experienced Social Worker: (Grade G9) – Learning Disabilities Team
Grade G9: £42,839 - £46,142 per annum
We are currently looking for three experienced Social Workers to join our well-established, highly experienced and supportive Community Learning Disabilities Team in Walsall.
This is an exciting opportunity to work within a strengths-based, person-centred service that supports adults with diagnosed learning disabilities to achieve positive outcomes, maximise independence, and live the lives they choose.
The successful candidates must hold a recognised Social Work qualification and be actively registered with Social Work England. Applicants should have a minimum of three years' post-qualification experience working within Adult Social Care.
The Role
As a Social Worker within our Community Learning Disabilities Team, you will:
- Undertake strengths-based assessments in accordance with the Care Act 2014.
- Complete mental capacity assessments in accordance with the Mental Capacity Act
- Develop, implement and review person-centred care and support plans.
- Work collaboratively with health colleagues, care providers, families and partner agencies.
- Lead safeguarding enquiries and contribute to positive risk management.
- Support adults with learning disabilities to promote choice, independence and wellbeing.
- Maintain high-quality recording and evidence-based practice in line with professional standards and local procedures.
