Experienced Social Worker – Community Learning Disability Team

Join our Community Learning Disability Team (CLDT) as an Experienced Social Worker and make a meaningful difference to the lives of adults with learning disabilities. We are looking to recruit two qualified Social Workers with 3–4 years’ post-qualification experience, ideally gained within a specialist Learning Disability setting. You will join a Health and Social Care integrated team, working collaboratively with colleagues, residents, families, carers and partner agencies to promote independence, wellbeing, choice and positive outcomes.

About the role

As an Experienced Social Worker, you will undertake a varied and rewarding caseload, including:

  • Completing strengths-based assessments of residents’ care and support needs.
  • Undertaking statutory reviews and ensuring support remains appropriate, proportionate and outcome-focused.
  • Promoting independence, choice and control for adults with learning disabilities.
  • Identifying and responding appropriately to safeguarding concerns, protecting vulnerable residents from abuse and neglect.
  • Working in partnership with residents, families, carers and professionals to develop effective support plans.
  • Developing proportionate and cost-effective care packages, ensuring resources are used appropriately.
  • Exploring alternative options and community resources to maximise independence and reduce reliance on formal services.
  • Assessing and clearly articulating risk, including the potential consequences where support is not provided.
  • Ensuring all work complies with relevant legislation, statutory duties, Council policies and procedures.
  • Working in accordance with the Knowledge and Skills Statements for Social Workers in Adult Services and the Professional Capabilities Framework (PCF).

What We Are Looking For

We are seeking experienced, motivated Social Workers who are committed to delivering high-quality, person-centred practice.

You Will Be Able To Demonstrate

  • Strengths-based and asset-based practice You will recognise and build on each resident’s strengths, abilities, resources, relationships and existing support networks.
  • Equality, Diversity and Inclusion (EDI) You will take account of individual circumstances, identity, culture and personal preferences, ensuring assessments and interventions are fair, inclusive and person-centred.
  • Outcome-focused practice You will clearly identify what support is intended to achieve and how it will improve the resident’s wellbeing, independence and quality of life.
  • Resident-focused decision making You will ensure the resident remains at the heart of the assessment and planning process, with a clear focus on the anticipated benefits and positive outcomes.
  • Evidence-based practice You will use clear and robust evidence to identify needs, risks and appropriate interventions, ensuring recommendations are well reasoned and proportionate.
  • Effective resource management You will have experience of developing care and support packages that are appropriate, proportionate and cost-effective, demonstrating why proposed support represents the best use of available resources.
  • Creative problem-solving You will consider a range of options, including community and informal resources, and clearly explain why the recommended approach is the most appropriate for the resident.
  • Risk awareness You will be confident in identifying, assessing and managing risk, including clearly articulating the potential consequences if identified support is not agreed.
