Adult’s Social Worker

Liquid Personnel is recruiting an Adult’s Social Worker to join its client’s Urgent & Emergency Care team, based in Nottingham. This is an excellent opportunity for a dedicated practitioner to work within a busy hospital-based service, supporting adults through discharge pathways and ensuring safe, timely, and person-centred transitions from hospital into the community.

Working across acute and sub-acute hospital settings, the successful candidate will be responsible for undertaking strengths-based assessments, coordinating discharge arrangements, and supporting individuals to maximise their independence whilst reducing reliance on long-term statutory services. You will work closely with health professionals, carers, families, community providers, and multidisciplinary partners to achieve the best possible outcomes for people requiring support following hospital admission.

This role requires an experienced Social Worker with strong knowledge of hospital discharge pathways, Discharge to Assess processes, the Care Act 2014, Mental Capacity Act 2005, Continuing Healthcare, and reablement services.

Responsibilities

  • Managing a caseload of adults requiring support through urgent care, hospital discharge, and Discharge to Assess pathways.
  • Undertaking Care Act 2014 assessments, reviews, and support planning to identify eligible needs and facilitate safe discharge arrangements.
  • Applying strengths-based practice to support individuals to maximise independence, resilience, and community connections.
  • Working within Discharge to Assess pathways and supporting adults through discharge pathways 0-3.
  • Undertaking Mental Capacity Assessments and contributing to Best Interest decision-making in accordance with the Mental Capacity Act 2005.
  • Assessing and managing risk effectively, making proportionate and defensible decisions to support positive outcomes.
  • Working closely with health professionals, therapists, discharge coordinators, carers, families, and community services to coordinate support.
  • Identifying opportunities for reablement, rehabilitation, and preventative interventions that reduce reliance on long-term services.
  • Supporting Continuing Healthcare processes and contributing to Decision Support Tool (DST) meetings where required.
  • Participating in multidisciplinary decision-making within Transfer of Care Hubs to support effective discharge planning.
  • Undertaking safeguarding interventions and responding appropriately to safeguarding concerns involving vulnerable adults.
  • Promoting timely discharge planning and ensuring individuals are discharged on or before their medically optimised discharge date wherever possible.
  • Maintaining accurate, timely, and legally compliant assessments, reports, support plans, and case records.
  • Working collaboratively with system partners to achieve positive outcomes and improve patient flow through hospital services.

Qualifications and Experience

  • Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, CQSW, CSS, or equivalent recognised Social Work qualification.
  • Registration: Must be registered with Social Work England.
  • Experience:
    • Significant post-qualified experience within Adult Social Care services.
    • Proven experience working within Hospital Discharge Teams, Urgent & Emergency Care Teams, Integrated Discharge Services, or Discharge to Assess pathways.
  • Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Adult Safeguarding legislation, NHS Continuing Healthcare, and associated statutory guidance.
  • Assessment Skills: Proven ability to complete complex assessments, reviews, support planning, and risk management interventions.
  • Discharge Planning Experience: Experience applying Discharge to Assess principles and supporting adults through discharge pathways 0-3.
  • Continuing Healthcare Experience: Knowledge of Continuing Healthcare processes and Decision Support Tool meetings.
  • Multi-Agency Working: Strong experience working collaboratively with NHS professionals, community services, care providers, carers, families, and partner agencies.
  • Communication Skills: Excellent communication, negotiation, report-writing, analytical, and decision-making skills.
  • Professional Practice: Commitment to strengths-based, person-centred practice and promoting independence, wellbeing, and positive outcomes.
  • Personal Attributes: Kind, compassionate, proactive, resilient, and able to work effectively under pressure whilst managing competing priorities.
