About the Role
This is an exciting opportunity to join Preparing for Adulthood (Adult Social Care), a multi-specialist team at Nottinghamshire County Council, focusing on assessment and support planning for young people transitioning into adulthood. You will work in a strength-based manner, aiming to make a real difference in the lives of young people. Your role will be pivotal in helping them achieve their aspirations during this crucial time.
As a Social Worker, Your Key Responsibilities Will Include
- Conducting assessments and developing support plans for young people.
- Collaborating with Children’s Social Care, Education, and Health services.
- Supporting young people in achieving their goals and outcomes.
- Ensuring compliance with adult legislation and safeguarding practices.
- Maintaining accurate records and producing reports.
- Participating in team discussions and service reviews.
- Undertaking health and safety risk assessments.

To Thrive In This Role, You Will Need
- A Social Work qualification (registered with Social Work England).
- Knowledge of community care services and relevant legislation, including the Care Act 2014.
- Proficiency in using IT applications for case management and reporting.
