Job Opportunity: Social Worker for Young Adult Transitions (14-25 Service)
Our client based in Bristol is seeking an experienced Social Worker to join their Young Adult Transitions (14-25 Service). This is an exciting opportunity for a dedicated practitioner to support young people aged 14-25 as they transition into adulthood, ensuring they receive the right support at the right time to maximise independence, wellbeing, and positive life outcomes.
The successful candidate will work with young people, their families, carers, education providers, health professionals, and partner agencies to deliver strengths-based, person-centred interventions. You will lead on support conversations, Care Act Assessments, support planning, and service commissioning, while promoting independence through community resources, assistive technology, equipment, and early intervention approaches.

Responsibilities
- Managing a caseload of young people aged 14-25 who require support as they transition from children's services into adulthood.
- Undertaking strengths-based support conversations to identify needs, aspirations, strengths, and desired outcomes.
- Completing Care Act Assessments and reviews in accordance with relevant legislation, policies, and best practice guidance.
- Developing person-centred support plans that promote independence, wellbeing, choice, and preparation for adulthood.
- Supporting young people and their carers to develop independent living skills and maximise opportunities for community participation.
- Identifying and promoting non-commissioned support options, community resources, assistive technology, and equipment to achieve positive outcomes and reduce reliance on formal services where appropriate.
- Leading on the commissioning and coordination of services to meet assessed eligible needs.
- Working within an Early Help and preventative framework, supporting young adults before needs escalate.
- Working collaboratively with education providers, health professionals, families, carers, housing services, and partner agencies to ensure coordinated support.
- Undertaking Mental Capacity Assessments where required and applying the principles of the Mental Capacity Act 2005.
- Assessing and managing risks, ensuring appropriate safeguarding measures are implemented where necessary.
- Supporting young people with a range of needs, including learning disabilities, physical disabilities, autism, mental health needs, and complex care requirements.
- Preparing clear and comprehensive assessments, reports, support plans, and case recordings.
- Participating in multi-disciplinary meetings, transition planning meetings, and professional forums to ensure effective partnership working.
- Maintaining accurate and up-to-date records in accordance with legislative, professional, and organisational standards.

Qualifications and Experience
- Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW.
- Registration: Must be registered with Social Work England.
- Experience: Significant post-qualified experience within Adult Social Care, Children's Services, or Transition Services.
- Transitions Experience: Experience supporting young people through the transition from children's to adult services.
- Legislation Knowledge: Strong understanding of the Care Act 2014, Children and Families Act 2014, Mental Capacity Act 2005, Adult Safeguarding legislation, and associated statutory guidance.
- Assessment Skills: Proven ability to complete strengths-based assessments, Care Act Assessments, reviews, and person-centred support planning.
- Early Help Experience: Experience working within preventative and early intervention models to promote independence and positive outcomes.
- Multi-Agency Working: Ability to work effectively with education, health, housing, children's services, families, carers, and partner organisations.
- Communication Skills: Excellent communication, relationship-building, report-writing, analytical, and decision-making skills.
- Young People Experience: Experience supporting young adults with disabilities, additional needs, and complex circumstances.
- Driving Licence: Full UK driving licence and access to a vehicle is desirable due to the community-based nature of the role.
