Job Summary
This post is based in the pharmacy, children's out-patient clinics and on the specialist ward at Leeds General Infirmary, although you may be required to work at other sites to support the service when necessary.
The main duties for the post holder, alongside those outlined in the job description are:
- To be part of the highly specialised clinical pharmacy service to paediatric liver and renal transplant recipients, primarily in an MDT out-patient setting, supporting medicines optimisation and patient centred care.
- To co-develop and manage the day-to-day homecare provision of immunosuppression for all paediatric solid-organ transplant patients across the region/North of England, linking with the homecare hub to create an effective and efficient service. This will also include communication with GPs and community pharmacists to ensure continuity of care across the interface.
- To work collaboratively with the paediatric liver and renal pharmacy team and wider MDT, to support medicines management of in-patients and develop the role of the Specialised Clinical Pharmacy Technician.
