Job Introduction

We are seeking a compassionate and dynamic Specialist Community Learning Disability Nurse (RNLD) to join our multidisciplinary Learning Disability Team in Swindon on a 6-month fixed-term maternity cover contract.
This is an exciting opportunity to support adults with diagnosed learning disabilities whose complex needs cannot be met through mainstream services. Working as part of a skilled and supportive multidisciplinary team, you will play a vital role in delivering specialist assessment, intervention and care coordination, helping individuals achieve the best possible outcomes and maintain their health, wellbeing and independence.
You will work closely with service users, families, carers and partner agencies, providing expert clinical support, promoting person-centred care and ensuring individuals receive the services and interventions they need. This role offers the opportunity to make a genuine difference to the lives of people with learning disabilities while contributing to the ongoing development of a high-quality community service.

Key Responsibilities
- Undertake comprehensive assessments, risk assessments and crisis planning to identify individual needs and support effective care delivery.
- Provide specialist advice, interventions and ongoing support to individuals with learning disabilities, as well as their families and carers.
- Manage a defined caseload, ensuring care is delivered safely, effectively and in line with individual needs and identified risks.
- Assess service user requirements and determine the most appropriate care pathway, including referral to mainstream services with reasonable adjustments where appropriate.
- Lead and contribute to multidisciplinary team meetings, interface meetings and complex case discussions to support coordinated care planning.
- Work collaboratively with a range of professionals, including nurses, occupational therapists, psychiatrists, psychologists and physiotherapists, to deliver high-quality, person-centred care.
- Develop effective partnerships with internal and external stakeholders to support positive outcomes and service accessibility.
- Contribute to service development, quality improvement initiatives and the ongoing evolution of the Learning Disability Service within the HCRG Care Group structure.
Personal Specification
Essential
- Registered Nurse (RNLD or RMN) with relevant professional registration.
- Experience of working within community services and/or learning disability and autism services.
- Experience of supporting individuals with complex needs.
- Experience of working with individuals whose behaviours may be challenging.
- Strong assessment, clinical decision-making and care planning skills.
- Ability to undertake risk assessments and develop effective care plans.
- Confident working independently and as part of a multidisciplinary team.
- Experience of leading meetings and contributing to multidisciplinary discussions.
- Commitment to delivering person-centred care and promoting accessibility for all individuals.
- Excellent communication and interpersonal skills.
Desirable
- Experience of working within a Learning Disability service.
- Knowledge of safeguarding legislation, policies and procedures.
- Understanding of non-medical interventions and positive behaviour support approaches.
- Advanced assessment skills and experience managing complex clinical presentations.
- Knowledge of relevant legislation and best practice relating to learning disabilities and autism.
