An Exciting Opportunity for a Therapist
An exciting opportunity has arisen for an enthusiastic therapist with acute NHS experience to join our Integrated Discharge Team (IDT) working in A&E and other in-patient assessment areas.
About the Role
The team works closely with acute and community services, providing a comprehensive assessment of patients to avoid admission. We are an integrated team supported by a social care practitioner, specialist nurses, and consultant geriatricians. The service operates over seven days, between 8am and 6pm and the successful candidate will be rostered regular days of work (Monday, Tuesday, and Wednesday preferred) within this.
Responsibilities
- Demonstrate a flexible approach to the assessment of older adults in crisis.
- Acknowledge the broad aspects of medical and social factors influencing frailty.
