Job Introduction
Are you an experienced Occupational Therapist looking for an opportunity to help shape a brand new service and make a lasting difference to people across Medway? HCRG Care Group is proud to have been chosen by Medway Council to deliver the Intermediate Care and Reablement Service from 1 October. As we launch this exciting new service, we are looking for a Specialist Occupational Therapist to join our growing team and play a key role in supporting people to recover, regain independence and remain in the place they call home. This is an exciting opportunity to join a service at the very beginning of its journey. Working alongside health and social care colleagues, you will help us develop and deliver innovative, person-centred rehabilitation and reablement pathways that improve outcomes for local residents and support our Home First approach.

Main Responsibilities
As a Specialist Occupational Therapist, you will provide specialist assessment, rehabilitation and reablement interventions for individuals presenting with a wide range of complex physical, medical, surgical and neurological conditions. Working autonomously across community and intermediate care settings, you will support people to maximise their independence and achieve their personal goals following illness, injury or hospital admission. You will undertake comprehensive functional assessments, develop personalised rehabilitation plans and work closely with patients, carers and multidisciplinary colleagues to deliver coordinated, person-centred care. Your role will include assessment for equipment and adaptations, supporting discharge planning and helping people safely return to, and remain within, their own homes wherever possible.
Alongside your clinical responsibilities, you will provide leadership and support across the therapy team, helping to coordinate rehabilitation and reablement provision, contribute to service development and ensure the delivery of high-quality care. You will work collaboratively with colleagues across health and social care to improve patient outcomes and shape the continued success of this new service.
Person Specification
Essential
- Degree or Diploma in Occupational Therapy
- Current HCPC registration
- Significant post-registration Occupational Therapy experience
- Experience working at Band 6 level or above
- Experience working within rehabilitation, intermediate care, community services or similar settings
- Experience managing complex clinical caseloads
- Strong understanding of rehabilitation, reablement and functional assessment
- Excellent clinical reasoning and decision-making skills
- Experience working within multidisciplinary teams
- Experience supporting, supervising or developing junior colleagues and students
- Excellent communication, organisation and prioritisation skills
- Full UK driving licence and access to a vehicle for work purposes
Desirable
- Experience working within Intermediate Care or Reablement services
- Community Occupational Therapy experience
- Experience undertaking home visits and environmental assessments
- Experience recommending equipment and adaptations
- Experience supporting service improvement initiatives
- Experience coordinating therapy provision or providing professional leadership
- Membership of the Royal College of Occupational Therapists
- Relevant postgraduate qualification or specialist training
