Are you an Occupational Therapist passionate about helping older people stay independent, safe and confident at home?
We’re looking for a Specialist Occupational Therapist to join our innovative Community Ageing Well Service (CAWS) on a fixed-term maternity cover.
Working as part of a supportive multidisciplinary team, you’ll use your specialist OT skills to support people living with frailty, complex health needs and long-term conditions. Your work will focus on what matters most to each person, helping them maintain their independence, function and quality of life.

In this role, you’ll:
- Carry out specialist occupational therapy assessments
- Develop personalised rehabilitation and treatment plans
- Recommend equipment, adaptations and moving and handling solutions
- Support falls prevention, frailty management and admission avoidance
- Work closely with GPs, social care, community rehabilitation and voluntary sector partners
- Share your expertise with colleagues, patients and carers
- Contribute to service development and quality improvement
