Job Introduction
Are you an experienced Physiotherapist looking for an opportunity to help shape a brand new service and make a lasting difference to people across Medway? HCRG Care Group is proud to have been chosen by Medway Council to deliver the Intermediate Care and Reablement Service from 1 October. As we launch this exciting new service, we are looking for a Specialist Physiotherapist to join our growing team and play a key role in supporting people to recover, regain independence and achieve the outcomes that matter most to them.
This is an exciting opportunity to join a service at the beginning of its journey. Working alongside colleagues across health and social care, you will help develop and deliver high-quality rehabilitation and reablement pathways that support a Home First approach, improve patient outcomes and enable people to live as independently as possible within their own homes and communities.
Package Description
You will be joining a passionate and supportive multidisciplinary team dedicated to helping people maximise their independence following illness, injury or hospital admission. As part of a newly mobilised service, you will have the opportunity to contribute ideas, influence service development and play a key role in shaping rehabilitation and reablement services across Medway.

Main Responsibilities
As a Specialist Physiotherapist, you will provide specialist assessment, treatment and rehabilitation for adults with a wide range of complex physical, medical, surgical and neurological conditions. Working autonomously across community and intermediate care settings, you will support people to achieve their rehabilitation goals, maximise their functional ability and maintain their independence.
You will manage a diverse and complex caseload, undertaking comprehensive assessments and developing personalised treatment plans that support recovery and reablement. Working closely with patients, carers and multidisciplinary colleagues, you will help facilitate safe and timely hospital discharge, reduce unnecessary admissions and support individuals to remain within their own homes wherever possible.
Alongside your clinical responsibilities, you will provide leadership and support across the therapy team, helping to coordinate rehabilitation provision and promote a consistent, person-centred approach to care. You will contribute to service development, quality improvement and the ongoing growth of this exciting new service, while supporting colleagues through supervision, teaching and shared learning.
Person Specification
Essential
- Degree or Diploma in Physiotherapy
- Current HCPC registration
- Significant post-registration Physiotherapy experience
- Experience working at Band 6 level or above
- Experience working within rehabilitation, intermediate care, community services or a similar environment
- Experience managing complex clinical caseloads
- Strong knowledge of physiotherapy assessment and treatment
- Excellent clinical reasoning and decision-making skills
- Knowledge of rehabilitation, reablement and Home First principles
- Experience working effectively within multidisciplinary teams
- Experience supporting or supervising junior colleagues and students
- Excellent communication, organisational and time management skills
- Full UK driving licence and access to a vehicle for work purposes
Desirable
- Community Physiotherapy experience
- Experience working within an Intermediate Care or Reablement service
- Experience participating in clinical audit, quality improvement or service development activities
- Experience coordinating therapy provision or acting in a clinical leadership capacity
- Membership of the Chartered Society of Physiotherapy
- Relevant postgraduate qualification or specialist training
- Knowledge of local health and social care services
