Key Duties and Responsibilities

The post holder will work autonomously within the Community Respiratory Service, utilising specialist clinical knowledge to assess, plan, implement, and evaluate care for patients with respiratory conditions while supporting service development and multidisciplinary working.

Key Responsibilities

• Undertake comprehensive specialist assessment, diagnosis, treatment, and review of patients with respiratory disease, including COPD, bronchiectasis, asthma overlap, emphysema, and ILD.
• Develop, implement, and evaluate evidence-based care management plans to support optimal patient outcomes.
• Contribute to the delivery of Community Pulmonary Rehabilitation programmes through patient education and exercise support.
• Assess patients for oxygen therapy requirements and undertake associated clinical assessments following appropriate training.
• Manage and prioritise a defined caseload using a case management approach to support effective care coordination and hospital avoidance.
• Communicate complex clinical information to patients, carers, and healthcare professionals, facilitating informed decision-making.
• Maintain accurate and contemporaneous clinical records and contribute to audit, governance, and quality improvement activities.
• Participate in multidisciplinary working, service development, teaching, supervision, and implementation of organisational policies and procedures.

Person Requirements

The successful candidate will possess the professional qualifications, knowledge, skills, and experience required to deliver specialist respiratory care within a community setting and contribute to service development.

Essential Criteria

• Registered Nurse or Physiotherapist educated to degree level.
• Current registration with the relevant professional body.
• Significant knowledge of long-term respiratory condition management.
• Knowledge of pharmacological interventions used in respiratory care.
• Experience caring for patients with respiratory conditions within a healthcare setting.
• Experience of multidisciplinary team working.
• Ability to undertake comprehensive clinical assessment and care planning.
• Effective written, verbal, and presentation skills, including group patient education.
• Competent use of electronic patient records and clinical IT systems.
• Ability to work autonomously and manage competing priorities.
• Commitment to continuing professional development and clinical governance.
• Full UK driving licence and access to a vehicle for work purposes.

Desirable Criteria

• COPD Diploma or equivalent specialist respiratory qualification.
• Independent/Non-Medical Prescribing qualification.
• Experience within community or integrated care services.
• Experience delivering education, training, or clinical supervision.
• Experience participating in service redesign, quality improvement, or innovation programmes.
