The salary scale for a Band 3 has been adjusted as part of the NHS Agenda for Change pay award for 2026/2027 and will be implemented in August 2026 backdated to 01 April 2026 where applicable.
Job Overview
- Responsibility for contemporaneous record keeping, identifying progress and plans in patients’ case notes
- Participation in consultant ward rounds and facilitating multidisciplinary meetings
- Complete accurate and timely discharge summaries, progress reports and ensure General Practitioners are informed of any impending discharges, changes of medication and death
- Assist consultants in the development of inpatient, outpatient and community-based services
- Assist in training and education of Foundation doctors and medical training grade doctors
- The dictation of Discharge Summaries
- Actively participate in multi-disciplinary audit, clinical effectiveness/health gain initiatives and Clinical Governance, taking lead in local clinical audit/Quality Improvement activities and developing research projects as appropriate.
The Post holder will be participating in the medical oncall Tier 3 rota and will be doing working at Registrar level managing acute medical take
Main duties of the job
The Health Board is developing an Older Persons Pathway to improve outcomes and patient experience across our system, from community to hospital. This work will begin to influence how care is delivered within the COTE Directorate and Community Medicine and Frailty Directorates, as the pathway is implemented. As this progresses, there will be a requirement for consultant roles and job plans to evolve to support the new model of care. The job plan outlined in this job description reflects current service requirements; however, further revisions to job plans should be expected as the pathway is implemented across the Health Board, in line with service need.
The Clinical Strategy of the Health Board is under constant development and the job holder may be required to undertake different duties agreed at the time of appointment.
