An Exciting Opportunity for a Specialty Doctor in Frailty and Older People’s Medicine
As part of our developing acute frailty model, you will play a vital role in delivering high-quality, patient-centred care for frail older adults. Working within a supportive, consultant-led multidisciplinary team, you will collaborate closely with consultants, resident doctors, nurses, therapists, and wider healthcare professionals to provide comprehensive assessment and management for patients across the Emergency Department (ED), Same Day Emergency Care (SDEC), and Care of the Elderly wards.
Role Responsibilities
- Provide specialist frailty assessment and management within the Emergency Department (ED) and Same Day Emergency Care (SDEC).
- Deliver rapid Comprehensive Geriatric Assessments.
- Identify appropriate alternatives to hospital admission.
- Support safe early discharge to improve patient outcomes and reduce unnecessary admissions and readmissions.
- Work collaboratively with multidisciplinary teams, community services, primary care clinicians, patients, and families to coordinate ongoing care and ensure continuity across healthcare settings.
- Participate in multidisciplinary meetings, clinical governance, audit, teaching, quality improvement initiatives, and undertake clinical procedures within your scope of practice.
- Support the education and development of resident doctors and medical students working on the Frailty ward.
