Staff Nurse: Permanent (37.5 hours)

We are currently recruiting a Staff Nurse to join our Frailty Unit, where we provide specialist care and assessment for older adults living with frailty, complex health needs, and multiple long-term conditions. The multidisciplinary team works collaboratively to deliver person-centred care, promote independence, support safe discharge planning, and improve patient outcomes.

Role Responsibilities:

• To assess, plan, implement and evaluate individualised, person-centred care for patients living with frailty, multiple comorbidities and complex health needs within your sphere of responsibility.
• To support the effective management and organisation of work within the Frailty Unit, ensuring patients receive safe, timely and coordinated care through multidisciplinary team working.
• To act at all times in accordance with the NMC Code, carrying out duties in line with Trust policies, procedures and professional standards.
• To contribute to comprehensive frailty assessments, care planning and discharge planning, promoting independence and supporting patients to achieve their optimum level of wellbeing.
• To work collaboratively with families, carers and the wider multidisciplinary team to ensure holistic care and smooth transitions between hospital and community services.
• To recognise and respond appropriately to the changing needs of frail older adults, escalating concerns and contributing to the prevention of deterioration where possible.
• To maintain accurate and contemporaneous patient records and communicate effectively with all members of the healthcare team.
• To participate in service development, quality improvement initiatives, training and continuing professional development to enhance the delivery of frailty care.
