What We’re All About
Our vibrant unit is dedicated to getting patients home safely and swiftly. With a cozy discharge lounge for patients awaiting their journey home, we focus on preventing de-conditioning and minimizing hospital stays.
Your Role
As a key member of our team, you’ll help plan our patient discharges, ensuring each patient’s needs are met and supported with a passion to embrace the patient’s day. Your enthusiasm will shine through as you engage patients in fun, uplifting activities like arts & crafts, card games, and even strolls outside, all designed to promote independence and well-being.

We need team players with excellent communication skills who can connect with individuals from all walks of life. You’ll collaborate closely with a dynamic multi-disciplinary team, ensuring a safe and efficient care environment for our patients, their families, and their carers.

Key Responsibilities
- Assess, plan, implement, and evaluate care programs under the guidance of senior staff.
