Slough OPMH Community Psychiatric Nurse Opportunity

Slough OPMH has an exciting opportunity for a Community Psychiatric Nurse who wishes to join our dynamic Home Treatment Crisis team. A flexible working shift pattern is just one of the attractions this opportunity will provide to you. You will provide high quality treatments and will work with the clinical team to take responsibility for the assessment and implementation of intensive care and treatment for people with mental health needs/dementia in the community.

You must have specialist knowledge of the care of older people with mental health problems and mental health crises. You will be required to travel within the community for the performance of your duties.

We are a friendly, dynamic and supportive team and you will have regular access to supervision and peer support in your role. You will be supported by our commitment to continuing personal development, and as such these are only two of many very exciting opportunities that Slough OPMH has to offer.

Responsibilities

  • Participating as a member of a multi-disciplinary team managing a caseload of patients
  • To undertake assessments of patients, involving their carers and family members in the patient's care and care plans.
  • To be responsible for the formulation, execution, and evaluation of care plans for a designated number of patients who may present with significant complexity, in consultation with the patient and the multi-disciplinary team.
  • The post holder will direct and supervise Support Workers and/or students.
  • Post holder will provide advice to other members of the service and agencies on the needs of service users.
  • To provide a fast and flexible response to care needs, responding to referrals in a timely manner and providing intensive treatment (up to four times daily).
  • To initiate and carry out the agreed treatment plan
