Staff Nurse – Mental Health Community Services

We have an exciting opportunity for a Band 5 Registered Mental Health Nurse (RMN) to join our supportive and friendly Older People's Mental Health Team based within the Newbury locality.

We welcome applications from clinicians who are passionate about working with older adults experiencing functional mental health difficulties, as well as people of all ages living with dementia, within a community setting.

This is a full-time post based at Hillcroft House, on the West Berkshire Community Hospital site.

The post is 37.5 hours per week, worked Monday to Friday, 9:00am to 5:00pm.

• Manage & take responsibility for a designated caseload, ensuring the delivery of high-quality, person-centred care.
• Undertake comprehensive mental health assessments in a variety of community settings, considering the individual's physical, psychological, social, cultural & spiritual needs.
• Develop, implement & review collaborative care & treatment plans in partnership with patients, carers, families & other professionals.
• Deliver evidence-based interventions & monitor their effectiveness, adapting care plans in response to changing needs & risks.
• Maintain effective communication & collaborative working relationships with primary care services, local authority colleagues, voluntary organisations & other partner agencies to ensure coordinated care.
• Actively contribute to multidisciplinary team discussions, care planning & decision-making to support positive outcomes for service users.
• Maintain accurate, timely & high-quality clinical records in accordance with professional standards, Trust policies & legal requirements.
• Ensure practice is delivered in line with Trust policies, professional codes of conduct, safeguarding procedures & relevant legislation.
• Take responsibility for maintaining professional registration & continuing professional development, contributing to a culture of learning within the team.
• Develop & maintain a specialist area of interest relevant to Older People's Mental Health Services by sharing of knowledge & best practice across the team.
