Exciting Opportunity for Newly Qualified Nurses

The Older Persons Assessment Unit is a 20-bedded front-door acute frailty assessment unit, providing rapid assessment and early intervention for older adults who present with acute or complex needs.
In addition, OPAU has Same Day Emergency Care (SDEC) capacity, including 7 trolleys and 2 chair spaces, enabling us to manage patients promptly, avoid unnecessary admissions, and support effective patient flow across the hospital.
Our multidisciplinary team works collaboratively to deliver high-quality, patient-centred care that promotes independence, dignity, and safe, timely discharge planning.

Main duties of the job
To Work According To The NMC Code Of Professional Conduct And Relevant Professional Guidelines As a Named Nurse Or Key Worker (with Facilitation) For a Defined Group Of Patients And Take Responsibility For
- The assessment of care and health education needs
- The development, implementation and evaluation of programmes of care including discharge planning for each patient.
To Gain Experience And Skills In
- Clinical practice
- Facilitation and teaching
- Management (as appropriate)
To work in accordance with the RUH Nursing Strategy and contribute towards achieving its objectives and maintain effective communication and participate in clinical supervision as appropriate and to teach and act as a facilitator/mentor/preceptor/ role model to less experienced staff.

Person specification
Essential criteria
- Registered Nurses with NMC Registration
- Demonstrates interest in patient flow
- Show awareness of the best practice
- Ability to mentor or support students nurses or junior staff.
Desirable criteria
- Demonstrate confidence in using electronic patient records
- Interest in developing career in elderly care
- Willingness to participate in service improvement
- Experience in end-of-life care or advanced care planning

Interview
Essential criteria
- Knowledge in age-related health conditions and their management, e.g. falls prevention; pressure ulcer prevention; polypharmacy
- Evidence of ongoing professional development.
- Demonstrates compassionate; kindness and civility attitude
Desirable criteria
- Previous experience in elderly care
- Completion of relevant clinical skills training
- Understanding of discharge planning in elderly care
