About Medserv
Medserv provides healthcare administration and billing services to healthcare providers across Ireland and the UK. Our software supports the day-to-day operation of medical practices, including billing, claims, payments, appointments, and patient administration.
The Role
This is Medserv's most senior hands-on individual contributor engineering role. You will play a leading role in the technical modernisation of the platform that supports our medical billing and practice management services.
You will remain substantially hands-on, while also helping shape architecture, technical standards, and how we move away from legacy systems over time.
Your initial focus will include defining the direction of our modern platform, strengthening engineering practices, and helping deliver new capabilities across claims, medical coding, billing automation, and practice management.
You will have genuine influence over technical decisions across the platform, while still spending most of your time designing and building software.
What You'll Do
- Design and build reliable, secure, and maintainable production software.
- Take ownership of complex technical problems from discovery through to production.
- Shape architecture, APIs, data models, and service boundaries.
- Modernise existing systems while continuing to support critical business operations.
- Write high-quality production code and review critical implementations.
- Improve automated testing, reliability, security, and observability.
- Mentor engineers and help establish consistent engineering standards.
- Work closely with Product and Engineering colleagues to turn business problems into practical technical solutions.

Essential Experience
- Experience operating at a senior technical level, taking ownership of complex production systems, and influencing technical decisions beyond your own individual delivery.
- Strong commercial PHP experience, ideally within a Laravel environment.
- Good knowledge of relational database design and MySQL.
- Strong experience designing production systems and APIs.
- Experience mentoring engineers or providing hands-on technical leadership.
Desirable Experience
- Frontend experience using React, Vue, or TypeScript.
- Modernising legacy systems while maintaining live services.
- Healthcare, billing, payments, or another regulated or operationally complex environment.
- Observability tools such as Sentry, Datadog, or Grafana.
- AI or LLM-assisted product capabilities in production.
- Experience building and operating production systems in AWS, Azure, or another cloud platform
You do not need to have previously held the title of Staff Engineer. We care more about the level you have operated at, the problems you have solved, and the technical influence you have had.

Additional Information
This is a remote role based in the UK and/or the Republic of Ireland. You will therefore be expected to attend our Dublin office for key planning sessions, design workshops, and collaborative activities.
