About the Role
We are seeking an experienced practitioner to join our Headington, Oxford service as a Support Coordinator for our high quality care and support for individuals with learning disabilities, physical disabilities, and additional mental health support needs to live exciting and independent lives.
Key Responsibilities
- Supervise an established team of staff providing person centered, assisting with the coordination and delivery of high quality outcome-focussed, person centred support to tenants.
- Promote fulfilling and meaningful lives for the people you will support.
- Work closely in partnership with people's families and other professionals.
- Coordinate, manage and coach a dedicated staff team.
- Deputise for the Service Manager.
Requirements
- Prior relevant experience in social care and an understanding of the needs of people with learning disabilities, physical disabilities, and additional mental health support needs.
- A positive and empowering approach with the ability to work collaboratively with staff and the families of service users and other agencies and professionals.
- Patient, professional and well organised with excellent communication skills, both verbal and written.
- Passion for quality and personalised service provision.
- Ability to liaise proactively and effectively with Care Managers and other members of the multi-disciplinary team.
- Minimum of NVQ3/H&SC Diploma Level 3 or equivalent and be willing to train to level 5 for the Support Coordinator position.

Working Conditions
- Flexible working with regular shifts, weekend working and occasional sleep-in duties (for which an allowance is paid).
