Role Overview
To deliver high quality person-centred support to customers living in supported accommodation service in accordance with the Person Centred principles, enabling clients to sustain their tenancies by promoting independence, freedom and choice.
Key Responsibilities:
- Assess referrals and engage with prospective clients using a person-centered approach.
- Liaise with health professionals, other agencies and colleagues as appropriate. Forge and maintain links with partner agencies and other key stakeholders as appropriate to ensure that good communication and co-operation is maintained.
- Ensure that prospective and current clients are fully aware of the support available, their financial obligations, rights and responsibilities under their tenancy/occupation arrangements and all related procedures.
- Agree and review client Independent Living Plans and Risk Assessments on a regular basis, deliver contracted hours of support to achieve the required outcomes.
- Report any concerns relating to safeguarding of vulnerable people using the correct procedures, attending case conferences and other meetings as required.
- High level of relationship skills: empathy, mutual acceptance, supportive and constructive challenge; positive expectation for the future.

More about you:
- A working knowledge and understanding of how to support people in achieving and maintaining independence essential.
- Housing or Support related qualification desirable.
- Experience of providing support to diverse client groups.
- Experience of working with clients experiencing substance misuse and mental health.
- Educated to GCSE level or equivalent in Maths and English (Grades A*- C).
- Ability to work under own initiative.
- Ability to network and build strong relationships with partner agencies, community and key stakeholders.
- Ability to use IT packages and management information systems.
- Ability to deal with distressed and aggressive clients.
- Driving Licence and use of own transport desirable.
