We are currently looking to recruit enthusiastic and motivated individuals, to join our fast-paced service provision team based in the Council’s Adult Social Care Operations department.
The focus of your role will be to work collaboratively with service users and their carers, internal hospital teams, assessment hubs, Client Financial Affairs, and commissioned and independent sector providers. You will be responsible for ensuring that services identified through assessment are put in place to meet an individual's care and support needs. This may include arranging direct payments, supporting carers' assessments, commissioning domiciliary care packages, and facilitating care home placements.
You will carry a caseload; therefore, you need to be organised and resilient but most of all you will care about the people we work with.

Requirements For This Job
- Experience of working in adult social care in a paid or unpaid capacity with a relevant service group.
- An understanding of legislation, procedures etc relating to the relevant service user group.
- Recognition of the differing physical and emotional needs pertaining to unpaid carers.
- High level of communication and negotiation skills.
- Ability to demonstrate literacy, numeracy and computer skills to complete assessments, correspondence and financial calculations.
- IT skills to be able to maintain accurate electronic records.
- Willingness to continue further personal training and development
- Willingness to travel between locations, either on foot or using public transport. Possession of a driving license and a car available for work may be desirable.
