Job Overview
This is an exciting opportunity for an experienced Support Time Recovery worker wishing to join the community mental health primary care team in Conwy. Applications are welcomed from enthusiastic practitioners who want to make a difference in primary care service provision. You will have excellent communication skills, be self-motivated, proactive, and committed to promoting innovation for service delivery.
The successful candidate will support and facilitate the organization of clinical activity, assist in the delivery of group and individual work while continuing to support and deliver the clinical activity as required. The post holder will be required to work independently and liaise with various third sector agencies and organizations as well as carers. Excellent skills in multidisciplinary working and the requirement to establish and maintain excellent links with GP practices are essential.
You will have excellent communication skills, and the ability to build and maintain effective working relationships, demonstrating a 'positive attitude'.
Main Duties of the Job
- Provide support and give time to an allocated group of service users, and promote their recovery supporting their return to a community environment, or supporting them to maintain their place in the community.
- Support service users to meet their identified needs and outcomes, paying attention to and respect their values, wishes, and aspirations to lead their lives in the way they choose, supporting them to access community resources and services, such as housing, benefits, leisure, education, and employment.

Qualifications
Essential criteria
- Certificate in Community Mental Health Care / NVQ Level 3 in care or agreed equivalent
- Good literacy and numerical skills
Desirable criteria
- Qualifications in Health, Social and Care
Experience
Essential criteria
- An awareness of issues affecting mental health service users
- Knowledge of approaches to working with mental health service users
- A knowledge of housing and welfare benefits, housing issues, and related employment and educational issues
Aptitude And Abilities
Essential criteria
- An ability to assess and evaluate individual needs to enable service users to make informed decisions
- An ability to analyse complex information concerning mental health issues and housing, finance issues
