Role Description
This is a full-time, on-site Support Worker role based in Wolverhampton. The Support Worker will provide day-to-day practical and emotional support to adults with complex needs, helping with daily living skills, medication prompts, and community engagement.

- Implementing person-centered support plans
- Promoting independence
- Supporting individuals to participate in meaningful activities at home and in the community
- Monitoring wellbeing
- Documenting in daily notes
- Escalating concerns
- Working closely with families and multidisciplinary teams to ensure consistent, safe, and high-quality support
- Contributing to maintaining a positive, inclusive, and safe environment
- Adhering to safeguarding, health and safety, and regulatory standards
Qualifications
- Experience working as a Support Worker or in a similar care or supported living role, with an understanding of complex needs and person-centered support.
- Strong interpersonal skills, including empathy, patience, and the ability to build trusting, professional relationships with individuals and families.
- Knowledge of safeguarding principles and practices, with a clear commitment to promoting safety, dignity, and rights.
- Effective communication skills, both verbal and written, including record-keeping and liaison with multidisciplinary teams.
- Understanding of positive behavior support and de-escalation techniques, or willingness to undertake training.
- Ability to work shifts, including evenings, weekends, and bank holidays, as required by the service.
- Right to work in the UK and satisfactory background checks in line with regulatory requirements.
