Supportive Care Physiotherapist

An exciting opportunity has arisen for an experienced and motivated physiotherapist to join the Cardiff and Vale Supportive Care Service.

The Supportive Care Service provides specialist multidisciplinary care for people living with advanced liver, renal, respiratory and cardiac disease who are likely to be in the last one to two years of life. Our aim is to reduce symptom burden, promote independence, enhance quality of life and support people to live as well as possible for as long as possible.

Working as part of a dynamic multidisciplinary team and alongside the Band 7 Physiotherapist, the successful candidate will play a key role in assessing and managing complex physical symptoms, delivering personalised rehabilitation, and supporting patients and their families to achieve what matters most to them.

This is an excellent opportunity for a physiotherapist with an interest in supportive care, long-term conditions, rehabilitation and palliative approaches to care.

• Provide specialist physiotherapy assessment and intervention for patients with advanced liver, renal, respiratory and cardiac conditions.
• Assess and manage symptoms including breathlessness, fatigue, reduced exercise tolerance, deconditioning, mobility problems and functional decline.
• Deliver personalised rehabilitation programmes to support independence, function and participation in meaningful activities.
• Work collaboratively with patients to establish realistic goals that reflect their priorities and preferences.
• Contribute to holistic multidisciplinary assessments and care planning.
• Support patients to self-manage symptoms and optimise physical wellbeing.
• Work closely with consultants, specialist nurses, occupational therapists, psychologists and other members of the multidisciplinary team.
• Participate in service development, audit, evaluation and quality improvement initiatives.
• Support the education and development of colleagues, students and support staff.
