An exciting opportunity has arisen for a Team Co-ordinator to join our dedicated team at Ings Grove House, a 40-bed Intermediate Care Service supporting adults to regain independence following illness, injury, surgery or hospital admission.
At Ings Grove House, we provide person-centred rehabilitation, reablement and assessment support, working closely with therapy teams, nursing services, social workers and other healthcare professionals to help people achieve their goals and return home safely wherever possible.
As a Team Co-ordinator, you will play a key role in the day-to-day leadership of the service. You will support staff on shift, promote high standards of care and ensure the service remains safe, effective and responsive to the needs of the people we support.

Key Responsibilities Include
- Leading and coordinating staff to ensure the smooth and safe operation of the service.
- Supporting individuals to achieve their rehabilitation and reablement goals.
- Supporting admissions, transfers and discharge planning processes.
- Promoting person-centred care, dignity, choice and independence.
- Supporting the safe administration and management of medicines, including audits and competency monitoring.
- Undertaking quality assurance audits and contributing to continuous service improvement.
- Supporting compliance with CQC standards, policies and regulatory requirements.
- Providing supervision, guidance and support to staff.
- Working in partnership with families, carers and multidisciplinary professionals.
- Supporting the Deputy Manager and Manager with service delivery, performance monitoring and improvement initiatives.
About You
You will have experience of working within a health and social care setting and be able to demonstrate strong leadership, communication and organisational skills. You will be passionate about delivering high-quality care, supporting staff development and achieving positive outcomes for the people who use our service.
