About the Role
The post holder will support service development, supervise and manage the CLDN team. The post holder will provide specialist health and behaviour assessments to children and young people who are referred into the service. Interventions will be delivered across different settings, including the home, school or clinic / hospital based. The post holder will work in partnership with children, young people, their families and carers and other health / social / educational professionals to achieve positive health outcomes for children and young people with learning disabilities.
The role provides clinical nursing expertise to children and young people referred to the team and be responsible for the management of a defined case load. The post holder will require strong reporting skills, good organisational skills and staff management skills.
Service Hours
The service operates Monday - Friday 0900-1700, excluding bank holidays.
Requirements
Applicants must hold a valid driver's license and have access to a car.
Responsibilities
- To provide a safe, responsive, efficient, timely, high-quality evidence-based nursing service to children and young people with learning disabilities, their families and carers.
- To be aware of, and participate in meeting the psychological, social, physical and spiritual needs of children and young people.
- Undertake a range of comprehensive assessments on children and young people with highly complex and challenging needs.
- Lead on the development and review of Trust policies and procedures relating to specialism.
- Participate in directorate / trust / regional meetings when required and deputise where requested.
