Job Overview
We are seeking a dependable and experienced Team Leader – Support Worker to join Yorkshire Community Care’s home support services. The post-holder will lead a small team delivering high-quality, person-centred care to adults with additional needs across home and community settings. This role combines hands-on support with supervisory responsibilities to ensure safe, responsive, and dignified care that promotes independence and positive outcomes.
Key Responsibilities
- Lead, motivate, and support a team of support workers, ensuring delivery of consistent, compassionate, person-centred care in line with individual care plans and organisational standards.
- Provide direct support to service users as required, including personal care, medication prompts/administration as delegated, meal preparation, and assistance with daily living activities.
- Coordinate daily tasks and rotas within the team to meet service demands, including allocation of visits, travel planning, and contingency cover for staff absence.
- Carry out regular supervision, coaching, and performance reviews with team members, identifying training needs and supporting professional development.
- Monitor care quality through spot-checks, care record reviews, and team debriefs, ensuring accurate and timely documentation using organisational systems.
- Promote positive relationships with service users, families, and external partners, acting as a point of contact for care-related enquiries and feedback.
- Identify, assess, and report safeguarding, health, or wellbeing concerns promptly to the appropriate manager or external professionals, following organisational procedures.
- Contribute to care planning, risk assessments, and any reviews to ensure person-centred approaches are current and effective.
- Support recruitment, induction, and training of new team members and participate in team meetings, shift handovers, and multi-disciplinary communication.
- Ensure compliance with infection prevention and control procedures, health and safety policies, and all statutory and organisational requirements.

Person Specification
- Proven experience in a support worker role with responsibility for supervising or leading others; experience in a community or complex care setting is desirable.
- Strong leadership and interpersonal skills with the ability to manage, motivate, and develop a team while maintaining a positive, person-centred culture.
- Excellent communication skills, both verbal and written, with experience of maintaining accurate records and using basic IT systems for rostering and reporting.
- Knowledge of safeguarding, risk assessment, and best practice in care delivery; ability to respond calmly and professionally in challenging situations.
- Compassionate, flexible, and organised, with good problem-solving skills and the ability to prioritise tasks and manage a dynamic workload.
- Willingness to undertake relevant training, supervision, and an enhanced DBS check as part of pre-employment checks.
- Valid UK driving licence and access to a vehicle may be required depending on service location and responsibilities; mileage allowance provided where applicable.
