We have an exciting new opportunity for a Team Manager to help our ongoing work on making improvements within our Discharge to Assess pathway alongside supporting the wider Hospital Social Care Team.

About The Team

The Hospital Social Care Team (HSCT) operates across three main hospital sites in Warwickshire and Coventry, delivering a fast-paced, responsive service that supports safe and timely discharge from hospital.

We work within the Discharge to Assess model, using a strengths-based approach to support individuals to regain independence and achieve positive outcomes following hospital admission.

Our multidisciplinary teams include Social Workers, Social Care Workers, Trusted Assessors and Business Support colleagues, working closely with health partners to ensure effective and person-centred discharge planning.

This is a supportive and collaborative environment, with a strong focus on teamwork, professional development and continuous service improvement.

About The Role

We are looking to recruit a part-time (22.5 hours per week) Team Manager to join the HSCT management team, with a key focus on leading and developing the Trusted Assessor function and supporting the Discharge to Assess (D2A) residential/block bed pathway.

This is a pivotal leadership role where you will:

  • Provide day-to-day management and oversight of Trusted Assessors, supporting workload management, practice consistency and progression of assessments
  • Lead on improving flow and performance within the D2A residential pathway, ensuring timely progression and effective use of block bed capacity
  • Work closely with health colleagues and system partners to reduce delays and support safe discharge planning
  • Maintain oversight of complex cases and support staff with decision-making in line with Care Act and Mental Capacity Act frameworks
  • Provide management cover across the HSCT, supporting wider teams and contributing to service resilience

You will be a visible and supportive leader, providing supervision, guidance and challenge to ensure high quality, strengths-based practice and positive outcomes for individuals.

As part of the HSCT management team, you will also contribute to service development and continuous improvement, helping to strengthen performance, consistency and workforce capability across the service.

Key Requirements

We are looking for an experienced and confident leader who can manage complexity and drive improvement within a fast-paced hospital environment.

You will have:

  • A recognised Social Work qualification and registration with Social Work England
  • Strong experience in adult social care, ideally within a hospital or discharge setting
  • Proven leadership and supervisory experience, with the ability to support and challenge staff to deliver high standards of practice
  • Experience of working within Discharge to Assess pathways or similar fast-paced environments
  • The ability to manage performance, oversee complex decision-making and improve service flow
  • Strong partnership working skills, particularly with health colleagues and multi-agency teams
  • Excellent organisational and communication skills, with the ability to balance competing priorities
  • A commitment to strengths-based, person-centred practice
  • Able to work across a 7-day rota, including one weekend every four weeks, to support a streamlined service and ensure timely hospital discharges. Candidates must be prepared to work weekends as part of the team’s commitment to responsive care.
  • Proficiency in IT systems – experience with Mosaic is desirable.
  • A full UK driving licence and access to transport is essential.
