Job Overview
This is an exciting opportunity to join the Gwynedd Learning Disability Community Health team at Betsi Cadwaladr University Health Board. This is a Band 7 Health Team Manager position within Gwynedd. The post holder will lead and manage the Gwynedd Community team, ensuring interfaces between the Local Authority and neighbouring services within the Division including community mental health teams, in-patient services, as well as other statutory agencies and the voluntary sector. Using a multi-disciplinary team approach, the post holder will develop the team so it works flexibly, proactively, and responsibly to develop and manage clinical care for service users in Gwynedd to support the needs of the service. The post holder will ensure that the team they manage delivers evidence-based, safe, effective interventions and clinical care, following the nursing process.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
Main duties of the job
The post holder will ensure that the team follows the service principles and strategy for people with a learning disability while delivering safe, effective, and high-quality specialist service in line with agreed standards. The post holder will have excellent communication skills and will be a visible and accessible manager to whom staff, patients, and their families can access for assistance, advice, and support. The post holder will monitor the interaction between staff and service users ensuring that they are treated with dignity, kindness, and compassion at all times.

You will be accountable for the performance management in the team relating to KPIs and outcome measures ensuring that fundamentals of care are in place and audits are completed where relevant. The post holder will be expected to communicate and receive highly complex and sensitive information and be able to analyse and interpret information and act appropriately where there may be barriers to acceptance. You will review and respond to Datix and safeguarding, workforce issues, and performance which will be reported to the service manager. The post holder will have a good understanding of the CHC process and also be part of the development of pathways and Maturity Matrix.
