NHS Lanarkshire is comprised of Acute Services (which currently provide hospital based services over 3 main sites), Corporate & Property & Support Services, North and South Lanarkshire Health and Social Care Partnerships which provide integrated primary healthcare and social care services to local communities and surrounding areas.

The Role

The Hospital at Home (Paediatrics) Team Co-ordinator is responsible for the day-to-day coordination and operational leadership of the Paediatric Hospital at Home service. The post holder ensures safe, effective and person-centred care is delivered to children and young people within their own homes, supporting admission avoidance, early supported discharge and integrated care pathways as an alternative to inpatient admission.

Please note this is a fixed term post for 12 months. Where a post only has temporary funding and an existing member of NHS Lanarkshire staff wishes to apply, this will be treated as a secondment. The employee MUST already have written agreement from their line manager to be released on a secondment before applying for the post.

In This Key Role, You Will

  • The post holder will be responsible for the line management of this integrated multi disciplinary team
  • Develop, implement and evaluate local policies and protocols to ensure clear lines of accountability and responsibility to ensure efficient and effective use of resources and service delivery, across all agencies and individuals providing care to patients in the community.
  • Provide and receive highly complex and sensitive information to support the operational management of the multi disciplinary team
  • Ensure implementation of local, national and regional policies, procedures and guidelines pertinent to all staff within the multi-disciplinary team in pursuit of the highest standard for the delivery of care.

What You'll Bring

  • Registered Nurse or Registered Allied Health Professional
  • Educated to Degree level
  • Evidence of personal contribution to service planning and redesign projects
  • Demonstrable knowledge of multi-disciplinary and inter-agency team working practices
