Temporary Care Coordinator
Location: Longmoor Road, Fazakerley
Pay: £15.00 per hour + holiday pay
Contract: Temporary (approximately 3 months)
Full time, office based
Start: As soon as possible
We're looking for an experienced and motivated Care Coordinator to join our team on a temporary basis. This is a great opportunity to make a real difference to the lives of the people we support while working within a passionate and dedicated team at Routes Healthcare.
About The Role
As a Temporary Care Coordinator, you will play a key role in the day-to-day coordination of care services, ensuring our clients receive high-quality, person-centred care. You'll work closely with carers, clients, and families to manage rotas, respond to changes in care needs, and keep everything running smoothly.

What You'll Be Doing
- Coordinating care packages and managing carer rotas to ensure full service coverage
- Acting as a point of contact for clients, their families, and care workers
- Responding promptly to changes in care needs, unplanned absences, and service gaps
- Supporting the recruitment, induction, and ongoing development of care staff
- Monitoring care delivery standards and escalating concerns where necessary
- Maintaining accurate records and documentation in line with company policy and regulatory requirements
- Building and maintaining positive relationships with commissioners and external stakeholders
- Contributing to a culture of continuous improvement within the team
What We're Looking For
- Previous experience in a care coordination or supervisory role within a care setting
- Strong organisational skills with the ability to prioritise and manage a varied workload
- Excellent communication skills, both written and verbal
- Confident using care management systems and general IT tools
- A compassionate, solutions-focused approach
- Ability to work flexibly and adapt quickly to changing priorities
- Knowledge of CQC standards and relevant legislation is desirable
