Help improve the physical health of people living with severe mental illness
Bradford District Care NHS Foundation Trust is seeking two motivated and compassionate Tobacco Dependency Advisors to join our Community Mental Health Teams.
This is an exciting opportunity to be part of a growing physical health programme focused on tackling one of the biggest causes of health inequalities for people living with severe mental illness. Smoking rates remain significantly higher among people accessing mental health services, contributing to poorer physical health and reduced life expectancy. As a Tobacco Dependency Advisor, you will play a key role in helping people make positive changes that improve both their physical and mental wellbeing.
Working as an integrated member of a Community Mental Health Team, you will provide evidence-based tobacco dependency treatment and behavioural support, helping people reduce or stop smoking through personalised interventions, harm reduction approaches and ongoing encouragement. You will work closely with nurses, social workers, psychologists, occupational therapists, pharmacy staff, peer support workers and medical colleagues to embed physical health improvement within routine mental healthcare.

- Deliver one-to-one tobacco dependency assessments and behavioural support.
- Develop personalised care plans and contribute to multidisciplinary care planning.
- Support people to access nicotine replacement therapy, stop smoking medications and harm reduction interventions.
- Provide advice on tobacco dependency, vaping and smoking cessation options.
- Promote healthy lifestyles and utilise a Making Every Contact Count approach. Support seamless transfer of care between mental health, primary care and community stop smoking services.
- Monitor outcomes, maintain accurate records and contribute to service evaluation and improvement.
- Act as a champion for smoke-free mental health services.
About You
You will be someone who:
- Is passionate about improving physical health outcomes and reducing health inequalities.
- Enjoys working directly with people to support behaviour change.
- Can build trusting relationships and have sensitive conversations with empathy and compassion.
- Works well both independently and as part of a multidisciplinary team.
- Has experience working within health, social care, community or mental health services.
- Has strong communication, organisational and digital skills.
- Holds, or is willing to undertake, Smoking Cessation Practitioner training
